The death of a Filipino citizen in Indonesia should be reported to and registered with the Philippine Consulate General in Manado depending on the province where the death occurred. The provincial jurisdiction of the Philippine Consulate General in Manado for consular matters is listed here.
After registration, the Philippine Consulate General transmits the Report of Death, through the Department of Foreign Affairs (DFA), to the Philippine Statistics Authority (PSA), which is the central repository of civil registry documents in the Philippines.
Related to the Report of Death, a consular mortuary certificate issued by a Philippine consular officer may be required by the relevant Philippine and/or Indonesian authorities if the remains of the deceased Filipino will be transported to the Philippines. Similarly, a certificate of no objection or equivalent document issued by the Philippine consular officer may be required by the host government in certain cases (i.e.: if the remains will be cremated or buried in Indonesia, etc.).
WHO MAY REPORT
- Immediate family member of the deceased
- Attendant at the hospital, clinic, or similar institution where the death occurred
- Any person who has direct knowledge of the facts of death
WHEN TO REPORT
Ideally, the informant should report the death of the deceased Filipino citizen to the Philippine Consulate General within one (1) year from the date of death to avoid delay in the registration. Delayed registration (beyond 1 year from the date of death) entails additional documentary requirements, procedures, and fees.
For those seeking delayed registration of death, a posting of public notice at the Consulate’s bulletin board for a period of not less than ten (10) days will be undertaken by the Consular Section. If after 10 days, no opposition to the registration was received by the Consulate and upon evaluation, the Consular Section is convinced of the veracity of the application, then the delayed registration of death will proceed.
PROCEDURE

- Submit the complete requirements below to the Consular Section of the Philippine Consulate General.
- After five (5) working days, claim the personal copy of the Report of Death (ROD) from the Philippine Consulate General, Consular Section.
Once the dispatch details of the ROD are provided by the Department of Foreign Affairs’ Office of Consular Affairs - Civil Registry Division (OCA-CRD), the Consulate General will notify the informant of the dispatch details through email. The dispatch details include the reference number used to obtain the Report of Death (ROD) from the Philippine Statistics Authority (PSA).

2.1. Original and five (5) photocopies of the Death Certificate issued by the Pencatatan Sipil of Indonesia along with a corresponding Apostille Certificate issued by the Indonesian Ministry of Law and Human Rights (KEMENKUMHAM).
2.2 If delayed registration, notarized affidavit of delayed registration executed by the informant.
- Original and 5 photocopies of the Philippine passport of the deceased.

PROCESSING PERIOD
The personal copy of Report of Death (ROD) for the applicant/informant will be available after five (5) working days.
For irregular cases of vital event registration that needs additional assessment and evaluation by the consular officer, the processing period will take longer than five working days. Depending on the nature of the facts of death that ought to be established, additional documents might be required by the consular officer on a case-to-case basis.
